0NLINE APPOINTMENT BOOKING

For existing patients of Dr Jerome and Dr Chawla

(We are not accepting any new patients at this moment)

STEPS TO CREATING AN ACCOUNT:

​1. Sign up by filling out all required details (full names please, especially double-barreled surnames, no nicknames).

2. Following signing up, you will receive an email requesting you to confirm your email address.

3. Log on at myhealthaccess, using the username and password that you chose in step 1.

3. Once logged on - Connect with our clinic from the clinic list, and then await approval by one of the clinic physicians (this can take up to 24hrs).

Once approved you will have an active account.

Thereafter, you will be able to book or cancel future appointments online using the 'my health access' button above or going directly to www.myhealthaccess.ca

To book appointments on your phone/ tablet search for 'myhealthaccess' app on the Android or Apple app store.

ONLINE APPOINTMENT BOOKING GUIDELINES:

Please note:

​1. If you do need to cancel an appointment, please do so at least 24hrs prior.

2. The online appointment booking system is only for standard routine appointment time slots. If you need, or have been advised to book a longer appointment please call the clinic.

(examples of longer appointments that are usually needed --> insurance forms, drivers medicals, complete physicals, etc)

3. Clinic Policy regarding 'No shows ' ('No shows' defined as - in person missed appointments with no prior cancellation notice to us)

A fee of $40 will be charged for NO Shows (for missed in person appointments, booked via the telephone or online system). ​​

4. While it is possible to book with the other office physicians if there is an urgent need (if they have available appointments), it is always advisable to book with your own physician to maintain continuity of medical care.

5. Unfortunately the online booking system doesn't allow for children to be registered for online booking. Should a child need an appointment, parents may book an online appointment using their account. This may be changed to the child when you check in on the day of the appointment. For parents trying to book an appointment for themselves and their children on the same day, please call instead.

6. One registered account per person ie. multiple people can not register under a single account.

7. In case of booking a follow up appointment to discuss scan results ( CT /MRI) , 24 BP monitoring results - it is advisable to ring first in order to make sure your results have been received by us, before coming in (as these results don't always come within the time frame mentioned at the testing facilities)

8. For patients of Dr Jerome - If you prefer a female physician for PAP / Breast/ pelvic exams, please book an appt with Dr Chawla, who has an interest in women's health.
